(Reuters) - A U.S. judge has accepted an agreement by BP Plc to plead guilty for its role in the Deepwater Horizon disaster and pay $4.5 billion in penalties for the worst offshore oil spill in U.S. history.
The April 2010 explosion on a rig in the Gulf of Mexico killed 11 workers. The mile-deep Macondo oil well then spewed 4.9 million barrels of oil into the Gulf over 87 days, fouling shorelines from Texas to Florida.
"The judge has accepted the plea," a court official said on Tuesday.
The company had said previously it would plead guilty to 11 felony counts related to the workers' deaths, a felony related to obstruction of Congress and two misdemeanors. It also faces five years' probation and the imposition of two monitors who will oversee its safety and ethics for the next four years.
With the plea agreement approved, BP has 60 days to send a remedial plan to the Department of Justice and the Environmental Protection Agency laying out how it plans to meet all of its stipulations.
The agencies will then review the plan and likely send it back to BP with proposed changes. The plan could go back and forth among all three parties before a final plan agreed to by all sides is reached.

It?s a fact of life: we?re going to die. God willing, we will be granted many long and happy years, and meet our end peacefully, surrounded by loved ones, ready to enter the presence of our Lord.
We are reminded far too often that death isn?t that cooperative. It can be sneaky and unkind. It is also indiscriminate, visiting the unprepared and the knowing in equal measure. It leaves grief, anger, confusion, and division in its wake, but that doesn?t stop it from ravaging families or entire communities.
Death makes us uncomfortable. We try to escape its inevitability through denial or vitamins. We believe we can transcend fate, as written in some of our great poetry ?Though lovers be lost, love shall not, and death shall have no dominion? ~ Dylan Thomas.
Thomas was absolutely right ? love ultimately conquers all, and our physical, earthly death is a new birth into eternal life. And yet, death is an unavoidable reality we have to face. This isn?t a topic I was excited to write about. We?ve just concluded our Christmas feasting; we?ve entered a new year of hope and promise, a Year of Faith. And yet, I feel very strongly the need to encourage you to prepare yourselves well for death.
Do all you can now in order to have a holy death. Pray, receive the sacraments, make your peace each day with God, beg the intercession of St. Joseph asking him to be with you in your moment of dying.
Do all you can now in order to have a peaceful death, whether you expect it or not.? Don?t leave forgiveness and reconciliation for tomorrow. Don?t leave the important things unsaid.
Do all you can now in order to have a quiet death. Don?t leave chaos and confusion behind you. Set your affairs in order now. Make provision for your children, settle your property, clean house. Make sure the pertinent people know where the paperwork is. Make your wishes known if you have strong feelings about your funeral and burial. If possible, make arrangements now for a burial plot. Ensure your finances are sorted in such a way that there will be no undue hardship for those who may need access to accounts, passwords, contracts.
This isn?t a morbid insistence that we all live in fear of an imminent death. A week before Christmas, the daughter of a friend died from cancer more quickly than anyone thought. Her husband and two young children had to make difficult decisions in the midst of their shock and grief. Not long ago, my friend?s mom died, again, unexpectedly. It?s taken my friend two years to sort through everything her mother left behind, all the collections, papers, files, all the ?just in case? stuff her mom couldn?t bring herself to get rid of. Instead, my friend had two years of physical labour and emotional turmoil, the constant decisions about whether to keep or where to dispose of the things that were inextricably linked to her mother?s memory.
This is not about guilt trips. I want to share my own experience. My dad died four years ago, on December 28. Two years prior to that, we discovered he had a condition that would ultimately end his life. It wasn?t until that September that we were told point-blank the bald fact: he was going to die, and fairly soon.? At the time, it was the cruelest news anyone could have given us. It turns out to have been the greatest blessing because we were able to do all those things I encouraged you to do above.
My dad was ready spiritually. He was completely at peace and was able to put himself with utter trust into the hands of Our Lady, and had no hesitation about where he was at with God. He was able to write notes for my mom about things he wanted her to know ? even how to use her mirrors when navigating the car. Never a terribly demonstrative or communicative man, he left me in no doubt as to his love for me, and I am so very, very grateful I was able to do the same for him. I didn?t want to let him go, but I could do so without regrets. He and mom bought their burial plots, selected his casket. He had his estate in order and talked it through with his executor ? and still the endless forms and phone calls and notifications after his death were difficult and seemingly endless. I can?t imagine having to do any of that while we were reeling with grief.
There is no need to be afraid of dying. We know our God is a loving, merciful Father. He has provisioned us with all we need to avail ourselves of His mercy, His grace. He has also blessed us with practicality and wisdom. Let?s make use of all His gifts to make sure that we and those we love, are prepared for a holy death.

The six bears that arrived this month at Animals Asia, an animal rescue center in China, had the grisly symptoms of inhumane ?bile milking.? Greenish bile dripped from open fistulas used to drain gall bladders; teeth were broken and rotted from gnawing on the bars of tiny cages.
Four of the bears have since had surgery to remove gall bladders damaged by years of unhygenic procedures to extract their bile, which is coveted for its purported medicinal properties. One bear?s swollen gall bladder was the size of a watermelon.
The latest batch of bears was rescued from an illegal farm by the Sichuan Forestry Department and joins 279 other bears at the center, near Chengdu in southwestern China. With luck, these bears will recover at the sanctuary. But thousands on farms, both legal and illegal, continue to suffer in wretched conditions, and countless others living in the wild across Asia are threatened by poaching and their illegal capture.
Bear bile contains a chemical called ursedeoxycholic acid, long used in traditional Chinese medicine to treat gallstones, liver problems and other ailments. There are an estimated 10,000 farmed bears in China, 3,000 in Vietnam, at least 1,000 in South Korea and others in Laos and Myanmar.
Tigers, rhinos and elephants are notoriously poached to satisfy high demand in Asia for their parts, which are falsely assumed to have medicinal properties. Experts warn that sun bears and Asiatic black bears, known colloquially as ?moon bears,? are on a similar route to endangerment, although their plight draws less media attention. ?No bears are extinct, but all Asian ones are threatened,? said Chris Shepherd, a conservation biologist and deputy regional director of the wildlife trade group Traffic who is based in Malaysia.
To address the threat, the demand for bear bile must be sharply reduced, Dr. Shepherd, a conservation biologist told hundreds of researchers at the International Conference on Bear Research and Management, an annual event held recently in New Delhi.
Reducing demand would require a multi-pronged effort, experts say. That would mean enforcing existing laws, arresting and prosecuting violators, promoting synthetic and herbal alternatives, and closing illegal farms.
Chinese celebrities like the actor Jackie Chan and the athlete Yao Ming have both spoken out against the bear bile industry to raise public awareness about poaching and the inhumane conditions typically found on farms. Bears often live for years in coffin-like cages in which they are unable to stand or turn around.
The bile is extracted through catheters inserted into the abdomen, with needles or by bringing the gall bladder to the skin?s surface, where it will leak bile if prodded.
Legal farming was conceived as a way of increasing the supply of bile to reduce the motivation for poaching wild bears, according to the International Union for Conservation of Nature. But there is no evidence that it has done so, it noted in a resolution passed last September, and there is concern among conservationists that it ?may be detrimental.?
The resolution also called on countries with legal bear farms to close down the illegal ones, to ensure that no wild bears are added to farms; to conduct research into bear bile substitutes (there are dozens of synthetic and herbal alternatives) and to conduct an independent peer-reviewed scientific analysis on whether farming protects wild bears.
Some groups argue that the increased supply of farmed bile has only exacerbated demand. ?Because a surplus of bear bile is being produced, bile is used in many non-medical products, like bear bile wine, shampoo, toothpaste and face masks,? Animals Asia says. Since bear farming began in China in the early 1980?s, bear bile has been aggressively promoted as a cure-all remedy for problems like hangovers, the group added.
In mainland China and Japan, domestic sales of bear bile are legal and theoretically under strict regulation as prescription products. But such sales are illegal in Cambodia, Malaysia, Myanmar, Singapore, Thailand and Vietnam, and the international trade is illegal as well.
Yet a 2011 report from Traffic indicated that bear bile products were on sale in traditional medicine outlets in 12 Asian countries and territories.
Nonprescription bear bile products like shampoo or toothpaste are illegal in China yet are readily available for purchase, conservationists say. Tourists from South Korea, a country that has decimated its own wild bear population, are major buyers in China and Vietnam even though taking bear bile products across borders is illegal under the Convention on International Trade in Endangered Species of Wild Flora and Fauna.
?Farms have drawn in bile consumers by creating a huge market ? bile is cheap,? said David Garshelis, a research scientist at the Minnesota Department of Natural Resources who is co-chairman of the I.U.C.N.?s bear specialist group.
In Vietnam, a milliliter of bile might sell for $3 to $6; about 100 milliliters can be extracted from a bear each day, according to Annemarie Weegenaar, bear and director of the veterinarian team at Animals Asia?s Vietnam center.
In four years, the I.U.C.N. is to issue a report on whether bear farms threaten wild populations. Meanwhile, demand appears to be spreading further afield in Asia and is now growing in Indonesia, largely as a result of demand from the Chinese and Korean communities there, said Gabriella Fredriksson, a conservation biologist based in Sumatra. A low-level poacher can sell a gall bladder from a bear caught in a simple snare and then killed for about $10.
So far the biggest threat to bears in Indonesia is loss of habitat from forest fires and the conversion of land to palm oil plantations. But in the last few years, poaching has increased, said Dr.
Fredriksson, who has been there 15 years.
She cautioned that bears in Indonesia could also become highly threatened. ?Fifty years ago, bears were doing well in Cambodia and Laos,? she said. ?Now there?s hardly any left.?

105-year-old Edythe Kirchmaier recently renewed her California drivers license (Facebook)There apparently isn?t much that can slow down Edythe Kirchmaier. The 105-year-old California resident made headlines on Monday when she passed her driving test ? continuing 86 years without a blemish on her driving record and maintaining her status as the state's oldest living driver.
"I just couldn't imagine myself without a car," Kirchmaier told FoxNews.com. "It just didn't feel very good."
And that?s far from the only bit of notoriety to crop up recently in Kirchmaier?s life. Facebook has declared her its most senior user, she's the University of Chicago's oldest living former student, last week she appeared on The Ellen Degeneres Show to celebrate her birthday and she has been a volunteer with the Direct Relief International (DRI) organization for 40 years.
Kirchmaier reportedly drives herself to DRI each week, where she leads a team of volunteers.
"I think I?m a pretty good driver," Kirchmaier told Fox. "I feel safe about getting my driver's license renewed because I?ve never had an accident."

Back in the day, coal miners always carried a canary with them in a small cage whenever they would go down in the mine. The canary is a very talkative bird, always singing and tweeting in a constant background noise. Canaries have one more characteristic important to a miner.
When there is bad air in the mine, the canary's sensitive lungs will cause it to drop over dead before the miners notice any shortness of breath. When the bird stops singing, the miners head for the surface -- fast. It is a life saving maneuver.
Here in 2013 as we start to implement the provisions of Obamacare and projections of physician shortages as high as 91,500 by 2020 from respected sources, I believe we have a similar "coal mine" situation in healthcare.
Unfortunately, physicians appear to be the canary in the mine.
Even before the pseudo-shortages of the Affordable Care Act, physicians were not in good shape. Symptomatic burnout is present in an average of one in three doctors on any given office day. A 2012 survey by the Physician's Foundation reported that 60 percent of doctors would retire today if they "had the means."
High patient volumes, the hassles of Electronic Medical Records (EMR), political uncertainty and changing reimbursement rates are all cited in surveys and online forums as new and increasing sources of stress for doctors. Online patient complaints always center on "the doctor seemed rushed, didn't listen or seem to care," which are classic signs of overwork and burnout.
The biggest new burnout threat may be Pay for Performance (P4P)
Healthcare organizations will soon be paid bonuses for reaching certain care quality indicators and patient satisfaction thresholds. Patient satisfaction is incredibly important and should be tracked and rewarded. At the same time, if care organizations simply use these reimbursement changes to dump more stress on the doctors they will actually block the doctor's ability to be present and care for their patients more effectively. Where is the tipping point, where the canary falls off the perch?
How did we get here in the first place?
Healthcare is a classic giving profession. Doctors and nurses learn early that patients come first. Our job is to serve them as best we can, no matter how tired or drained we may be feeling. Their needs come before ours. Simply acknowledging our own needs is a challenge. Finding time and tools to get them met is a struggle for most physicians.
Many healthcare organizations systemize this self care blind spot
It is rare for the physicians and staff to appear in the Mission Statement of a care organization. The patients and care quality will always be front and center in the mission. Any mention of the providers and staff is very rare.
[Take a quick look at your organization's Mission Statement. Does it mention provider or staff health, wellness and satisfaction? If so, is your organization actually doing something about it?]
This creates a double bind where the providers are challenged to get their needs met and their organizations fail to acknowledge we have any in the first place.
The last American sweatshop
As a result, many healthcare workplaces feel like a classic sweatshop environment. There are too many patients to be seen, multiple systems come between the doctor and patient (EMR for instance) there are quotas to be made and no regard whatsoever is paid to the health, wellness or stress levels of the physicians and staff.
The workplace resembles a mine. We are waiting for the physician/canary to drop.
In my work with doctors with career threatening burnout, it is common to hear of hospital departments and group practices where the entire staff is just barely making it through their work week.
Thank god for work hardening
The only reason these toxic workplaces don't implode is the stamina and work ethic of the doctors and staff in the system. On one level, residency is a work hardening program for physicians. We can take a tremendous licking and keep on seeing patients. Our organizations know this and just pile it on. The leaders count on the doctors to carry any load they pile on while taking no responsibility for the stress involved. It is just like putting a canary in a shaft where you know the air is bad and hoping it survives.
Everyone loses
Tragically, unless your group actually monitors for physician health and satisfaction, the toll burnout exacts on the doctors and their patients remains invisible.
Research shows burnout has a pervasive negative effect on every aspect of a physician's practice and life. Physician burnout has been linked to
- Lower quality of care and lower patient satisfaction rates
- Higher rates of medical errors and malpractice risk
- Higher physician and staff turnover
- Higher levels of divorce, alcohol and drug use and suicide for the physician - our Canary.
So how much longer can we simply pile more and more stress on the doctors? When will this canary be stretched beyond its limit and simply topple off its perch and crash to the bottom of the cage? Does anyone care or is this something doctors just have to take care of on their own?
There is a different path available to healthcare organizations.
This path results in a win:win:win situation.
The key is to stop taking the canary down the mineshaft in the first place. Focus the same amount of effort on creating a healthy environment for your physicians and staff as you do on patient satisfaction for one simple reason.
Your doctors cannot produce consistently high patient satisfaction scores if they are not consistently healthy and happy at work.
Happy doctors naturally have happier patients. Put the canary in a sunny window and change the papers in the bottom of her cage. This is a fundamental shift with positive consequences for everyone in healthcare.
The most successful organizations in the near future will be those that acknowledge the universal presence of physician stress and burnout.
There are hundreds of proven ways for both physicians and organizations to make real and lasting changes in the physician experience as soon as this fundamental shift is recognized.
I also predict this type of organization will be rare
Leadership must stop simply loading on more work responsibilities and cracking the whip. Organizations must be committed to caring about the doctors and staff and constantly focused on creating a healthier and more efficient workplace.
With this new priority of physician wellness, these same organizations will be able to activate the power of a physician staff that is balanced and healthy, with their physical, emotional and spiritual needs met. This is the natural foundation for consistent quality care and patient satisfaction.
Just imagine a work environment ...
... where you enjoy your work team and the practice environment, the systems work to let you care for patients smoothly and effectively and you know that your leadership "has your back" and really cares about the quality of your experience as a member of the group. What would that be like? What quality of care would doctors and staff in that organization offer to their patients naturally and automatically.
Our healthcare workplaces don't have feel like a coal mine. We don't have to sit around and wait for the canary to drop.

(Reuters) - Sci-fi filmmaker J.J. Abrams will direct "Star Wars: Episode VII," Walt Disney Co said after days of speculation, giving hope to many long-suffering fans who were disappointed by the last three installments in the iconic franchise.
The announcement was greeted with celebrations on online networks by the films' army of enthusiasts who have already watched Abrams rescue the aging "Star Trek" series with a high-grossing prequel in 2009.
Disney said late on Friday Abrams would work under the leadership of producer Kathleen Kennedy, the former president of Lucasfilm, and the script would be penned by Oscar-winning writer Michael Arndt.
"J.J. is the perfect director to helm this. Beyond having such great instincts as a filmmaker, he has an intuitive understanding of this franchise," Kennedy said in a statement on starwars.com.
Fans were equally enthusiastic. "JJ Abrams to the rescue!!! Yes!! #starwarsVII," wrote Jonny Radtke ?(@JonnyRadtke) on Twitter.
"Great news about JJ Abrams directing Star Wars. Might just rescue the brand...," added Alastair Brookshaw ?(@albrookshaw).
Rumors that Abrams, one of Hollywood's most successful directors and producers, would take stewardship of the films filled industry publications and online forums over the past week.
The 46-year-old made his name with TV shows "Alias" and "Lost" and earned his stripes as a director of effects-laden blockbusters with "Super 8," "Mission: Impossible III" and another widely expected Star Trek film "Star Trek Into Darkness," due out later this year.
The Star Wars franchise, which was created by George Lucas' Lucasfilms Ltd, has grossed more than $4.4 billion at the global box office since the first film was released in 1977, making it the third most successful movie property after the "Harry Potter" and "James Bond" series.
Star Wars characters such as Princess Leia, Luke Skywalker and the anti-hero, Darth Vader, have become a staple part of pop culture, along with the catch phrase "May the force be with you".
The Star Wars films were acquired by Disney after they bought Lucasfilm for $4.05 billion in October 2012. They announced then that three new installments would be made, starting in 2015.
Lucas, 68, gave Abrams his blessing on Starwars.com, saying the director was "an ideal choice to direct the new Star Wars film and the legacy couldn't be in better hands".
Abrams said his new role was "an absolute honor."
Disney had previously said Lucas would remain a creative consultant on the series.
Lucas himself said in October he had story treatments for Episodes 7, 8 and 9, which he would hand over to Kennedy, who assured him that she would adhere to his ideas.

On the inaugural stage built on the west front of the U.S. Capitol, a calm silence hushed over the crowd Monday as President Obama began his inaugural address, "Vice President Biden, Mr. Chief Justice, Members of the United States Congress, distinguished guests?"
Close to one million spectators listened quietly as the president's oratory emanated from loud speakers placed on the National Mall and echoed off the historic marble buildings and Smithsonian museums. Most of the audience was respectful, cheering the president as he recited the principles that will guide his second term.
But Rives Miller Grogan, an anti-abortion protestor, climbed high into a tree to unleash a lengthy tirade, refusing to yield to the president.
Grogan's incessant shouting annoyed many in the crowd standing in the grass below him as well as thousands of others within an audible distance of his perch. U.S. Capitol Police tried to coax him down, but Grogan climbed even higher, out of reach of ladders. Officers were compelled to wait him out.
Ultimately, after about five hours, Grogan climbed down and was arrested by U.S. Capitol Police. He was charged with a misdemeanor for illegally climbing a tree on federal property, and contempt of court for violating a Jan. 16 order to stay away from the U.S. Capitol. Grogan has pleaded not guilty.
It's not the first time he's been arrested for disrupting events in Washington. Grogan was arrested Jan. 16 and charged with disorderly conduct for an outburst in the House chamber after lawmakers had finished reading the Constitution. He was also pulled from the Senate chamber Jan. 2 for erupting into protest, blaming the Newtown elementary school massacre on abortion. Grogan's record in the District of Columbia dates back to 2006 when he was charged for simple assault and using objectionable language in the Supreme Court building.
This same protestor ran onto the field at during the MLB playoffs last October during a National League Division Series playoff game between the Cincinnati Reds and the San Francisco Giants, which Mitt Romney attended. That time he held an "Abortion is Sin" placard. Grogan also heckled the president at a campaign rally last November in Lima, Ohio, where he was dragged out of the balcony for yelling during the president's speech.
Now, D.C. superior court judge Karen Howze has had enough and banned Grogan from entering the District of Columbia, raising the specter of a violation of his constitutional right to free speech.
"I don't know why they would ban me from all of Washington," Grogan told the Washington Post. "They are totally suppressing my freedom of speech."
Grogan, who resides in California, has a court appearance scheduled on Feb. 25 for ascertainment of counsel, for which he will be permitted to enter the District in order to attend.

A federal judge in Chicago sentenced an American citizen to 35 years in prison on Thursday for his role in providing surveillance information and videos laying the groundwork for the 2008 terror attacks in Mumbai, India, that left more than 160 dead and hundreds wounded.
David Coleman Headley, a US citizen of Pakistani heritage, was arrested in October 2009. He agreed shortly afterward to cooperate with US investigators and intelligence officials, and he testified against one of his fellow co-conspirators.
Among other information, Mr. Headley told US officials of a link between the terror operation in India and Pakistan?s intelligence service, the ISI. He identified his ISI contact as ?Major Iqbal,? according to court documents.

?Major Iqbal? helped plan and fund the Mumbai attacks, he said.
In light of his cooperation, prosecutors did not seek the death penalty for Headley. In addition, instead of a life sentence, prosecutors urged US District Judge Harry Leinenweber to impose a 30 to 35-year prison term.
During the sentencing hearing, Judge Leinenweber called Headley a ?terrorist? and rejected suggestions he had reformed his life.

?I don?t have any faith in Mr. Headley when he says he?s a changed person and believes in the American way of life,? the judge said, according to the Associated Press.
In their sentencing memorandum, prosecutors acknowledged that Headley played ?an essential role in the planning of a horrific terrorist attack.?
?There is little question that life imprisonment would be an appropriate punishment for Headley?s incredibly serious crimes,? they said. But they added that his extensive cooperation had been of ?significant value? to US anti-terror efforts.
Headley, 52, is unlikely to emerge from prison until he is well into his 80s.
Headley pleaded guilty in March 2010 to all 12 counts in his indictment. The charges included conspiracy to bomb public places in India, conspiracy to murder and maim persons in India, and six counts of aiding and abetting the murder of US citizens in India.
Among the dead were six American citizens.
Headley attended a series of training camps in Pakistan in 2002 and 2003. He later decided to change his name from Daood Gilani to David Headley to allow him to conceal his Pakistani ties and portray himself as an American in India.
Headley told investigators that he made five trips to Mumbai in 2006, 2007, and 2008 to survey and videotape potential targets for a planned assault team from the Pakistani terror group Lashkar-e-Taiba.
At one point he used a GPS device to pinpoint potential landing spots for the assault team along the shores of Mumbai harbor.
The attacks were carried out from Nov. 26 to Nov. 28, 2009 by 10 men trained by Lashkar-e-Taiba. They were armed with assault rifles, grenades, and improvised explosive devices, and they killed indiscriminately.
Targets included the Taj Mahal Hotel, Oberoi Hotel, the Leopold Caf?, a Jewish center called the Chabad House, and the train station. Headley had provided surveillance of each of those targets.
Four months after the attacks, Headley returned to India to conduct surveillance of India?s National Defense College in Delhi and Chabad Houses in several other cities, officials said.
Headley was also asked to provide surveillance in advance of planned attacks on the Danish newspaper Morgenavisen Jyllands-Posten, which had published a controversial cartoon depicting the Prophet Mohammed in a derogatory light. The action prompted calls from Islamic militants ? including members of Al Qaeda ? for retaliation.
Headley traveled to Denmark in January 2009 and produced videos that he turned over to a Lashkar member later that month during a trip to Pakistan. He returned to Europe in July and August 2009 and produced 13 additional surveillance videos in Denmark.
Headley was arrested at Chicago?s O?Hare International Airport in Oct. 2009 while preparing to board a flight to Pakistan to deliver the 13 videos.
As part of his cooperation he testified at the June 2011 trial of alleged co-conspirator Tahawwur Rana of Chicago. Mr. Rana was convicted of providing material support to the Denmark plot and providing material support to Lashkar.
He was sentenced last week to 14 years in prison.

The Pines had long teetered on the brink of a shutdown, but the UHS takeover of the facility appears to have erased what standards had been put in place. A short time after the facility fell under UHS control in mid-November, it earned serious punitive sanctions. Two months into the company's tenure, a sense of lawlessness pervaded the facility, according to a review of documents obtained by The Huffington Post through a public records request.North Carolina, which had sent more than 100 kids to The Pines, stopped doing so this past spring, when that state's Division of Medical Assistance, along with other agencies, found widespread and systemic breakdowns in how the facility treated its children, according to the documents. In mid-April, the state concluded that it had to pull all 140 or so of its children -- including T. -- out of The Pines, according to email records.
Virginia has since barred new admissions and slapped the facility with a provisional license. According to the documents, Virginia inspectors found that Pines staff had been caught watching a pornographic DVD with residents, that one resident admitted to selling drugs and buying drugs from a Pines employee and that records concerning the care of one resident had been "fabricated."
In a statement released to The Huffington Post, Universal Health Services defended its practices: "The Pines management team is continually reviewing clinical programming, procedures and staff training to enhance the provision of safe, effective, and patient-centered treatment," the company statement reads. "The Pines is actively addressing any and all concerns relating to the treatment of our residents."

Time to get out the calendars and mark the date: "Mad Men" will return for its sixth -- and presumed penultimate -- season on April 7, AMC has confirmed.
The premiere episode will last for two hours, the network said, and will be written by show creator Matthew Weiner. That episode will be directed by Scott Hornbacher; Jon Hamm, who plays Don Draper in the show, will pull double duty and direct the season's second episode.
Few other details are available, but Weiner, who spoke with the New York Times on Tuesday dropped some hints. "It will advance in time, as it does," he said, but wouldn't specify what year the show would be based in. "We're coming off a period in Don's life where he's trying to normalize, and trying to have this relationship -- a real relationship with this woman that he fell in love with."
The two-hour opener, he added, was requested by the network. "It worked well last year and they wanted to try it again," he said. "For me, I was like, well, I don't have the same set of problems (to resolve with the characters), but I do have a way to start the story with a movie." He promised "cliffhanger elements" to that first episode and said it would "foreshadow a lot (of) what the season is about."

PHOENIX (AP) ? Justin Upton is having a family reunion in Atlanta.
Arizona traded its star right fielder to the Braves on Thursday in a seven-player deal that sent former All-Star infielder Martin Prado to the Diamondbacks.
For the first time since he was a high school freshman, Upton will have older brother B.J. Upton as a teammate.
The brothers combine with Jason Heyward, who won a Gold Glove in 2012, in an outfield potentially packed with power and speed.
"If we push ourselves to the next level, I feel with the extra push from each other there's no question we can be the best outfield in baseball," Justin Upton said in a telephone interview. "I'm not going to give us that label until we prove it."
The Braves, who also get third baseman Chris Johnson, are giving up one of their top pitching prospects, Randall Delgado, and three minor leaguers in the deal.
They are right-hander Zeke Spruill, shortstop Nick Ahmed and first baseman Brandon Drury.
Prado, projected to play third base for the Diamondbacks, can become a free agent after this season, but Arizona general manager Kevin Towers said he already was working with Prado's agent on a long-term deal.
B.J. Upton, 28, signed a five-year, $75.25 million contract with Atlanta in November.
Justin Upton, who has five full seasons in the majors but is just 25, said he already got tips from his brother from afar. Now there will be more chances for the two to help each other.
"I think from that standpoint it will be good, but I think more than anything being able to show up at the ballpark genuinely excited every day and have that energy," he said. "The more energy you can bring from the start every day, it makes you a better player."
Braves general manager Frank Wren said he expects the brothers will push each other.
"I do think it will drive them," Wren said. "We've been looking for that young dynamic, right-handed, power-hitting outfielder that can hit in the middle of the lineup and makes that other team think a little bit."
The younger Upton, who has three years and $38.5 million left on his contract, had been the subject of trade speculation throughout the offseason and vetoed a trade to the Seattle Mariners.
Upton had his ups and downs in Arizona, and Towers believes the change of scenery will benefit the young player, who was just 19 when he came to the majors.
"The expectations were through the roof on him," Towers said. "When the team struggled, it seemed like it was always because of Justin. That's hard. We're human beings. It's hard to take when you're a young individual trying to establish yourself."
Now, Towers said, Upton has "an opportunity to go to a different place to where he's going to fit in with some star players over there, where he's just kind of a piece of the puzzle versus kind of the centerpiece and the big piece of the puzzle.
"I think some pressure will be off of him."
Arizona manager Kurt Gibson, his left arm in a sling after shoulder surgery, said he sent Upton a long text wishing him the best.
"I said, 'You must be ecstatic to be able to play with your brother.' I could certainly understand that. It's got to be a great thrill for him."
Gibson said Upton reminded him of himself as a player.
"I had some things that happened with my manager Sparky Anderson over the years where maybe we butted heads a little bit and I didn't agree with him," Gibson said, "but when it was all said and done it all made sense and it was for me to become a better person and a better ballplayer. I would say similar things have happened with me and Justin throughout our association. I know he respects me and I respect him."
Prado, who joins fellow Venezuelan Miguel Montero in Arizona, made the All-Star team in 2010 as a second baseman and played mostly in left field last season. Prado was projected to move to third base for the Braves this season to replace the retired Chipper Jones.
Arizona had accumulated a glut of outfielders, signing Cody Ross as a free agent last month. Upton, an All-Star pick in 2009 and 2011, was the most marketable.
Upton, the No. 1 overall pick in the 2005 draft, has played five full major league seasons. Last season, he hit .280 with 17 home runs and 67 RBIs and a career-high 107 runs.
In 2011, while helping Arizona win the NL West, Upton hit .289 with career-best totals of 31 home runs and 88 RBIs and finished fourth in NL MVP voting. Overall, he's a career .278 hitter with 108 home runs, 739 hits, 147 doubles and 80 stolen bases.
Upton said he hopes to become a more consistent player in Atlanta.
"I've had a couple good years where I put up good numbers and my goal all along has been to put those years up consistently," he said. "I think now I'm in a position with Atlanta ... we can really feed off each other throughout that lineup to try to get everybody to that consistent production rate."
Delgado, a 23-year-old right-hander, had been expected to challenge for a spot in the Braves rotation. His acquisition bolsters the young pitching depth in Arizona depleted by the trade of Trevor Bauer. Delgado was 4-9 with a 4.37 ERA in 18 appearances, 17 as a starter, for the Braves. Rated Atlanta's No. 3 prospect by Baseball America the past two years, he was 4-3 with a 4.06 ERA with Triple-A Gwinnett last season.
The trade leaves Arizona with three veteran outfielders ? Ross, Jason Kubel and Gerardo Parra ? along with two youngsters the team feels are ready for the majors ? Adam Eaton and A.J. Pollock. Towers projects Eaton as a center fielder and a prototypical leadoff hitter.
Towers said the addition of Ross and a top contact hitter in Prado changes the character of the club.
"I would say we're going to be a little different club," he said. "I think we can still hit home runs, but I think the last couple of years we've relied too much on the long ball. If you look at our record, those days that we didn't homer, we usually didn't win."
